Clark is seeking a Registered Nurse to be the Assistant Director of Nursing / MDS Coordinator for our 5-Star Rated, 39-bed Skilled Nursing facility. We are looking for dynamic, energetic, organized, and efficient professional with experience with MDS. The successful candidate will foster an environment that will lead to outstanding outcomes for all of our patients. Supervisory experience in a geriatric long-term care facility is preferred

Job Responsibilities:

Some of the essential job functions are listed below (this list is not all-inclusive):

  • Plan, develop, organize, implement, evaluate and direct the nursing services, in accordance with current rules, regulations and guidelines that govern long term care, in collaboration with the Director of Nursing
  • Ensure delivery of compassionate quality care and nursing supervision
  • Monitor care to ensure that residents are treated with kindness, dignity, and respect
  • Work in collaboration with the DON to develop, maintain, and implement nursing policies and procedures that conform to current standards of nursing practice, facility philosophy, and operational policies while maintaining compliance with state and federal laws and regulations
  • Participate in admissions process
  • Participate in surveys / inspections made by authorized government agencies
  • Participate in weekend supervisory rotation
  • Regulate and maintain the MDS / PPS program and completion schedule


  • Must have an active RN license
  • One to three years of experience working in a Skilled Nursing / long-term care environment
  • Experience working with persons with dementia preferred
  • Ability to communicate effectively, both verbally and in writing
  • Excellent interpersonal and communication skills
  • Clear understanding of person centered care
  • Teamwork oriented
  • Good organizational and planning skills
  • Attention to detail
  • Ability to work under tight guidelines
  • Participation in weekend supervisory rotation
